Welcome to our practice. It is our goal to provide high quality, efficient and courteous care to our patients. It is our goal to see each patient in a timely manner and to avoid any rush to our patients. As a new patient, we do ask that you come prepared for your visit. You will need to provide appropriate

       
  • Personal information
  • Insurance information
  • Your personal medical history
  • Current and accurate listing of medications (including dosages)

It is important that we are aware of your medical history, surgical history as well as your medications (including vitamins and herbs), so that we can prepare for any surgical procedure that you choose to undertake. We are aware that it is frustrating supplying this information to all of your physicians, but your health and safety is extremely important to us.

It is important that you  bring a list of medications. If you  download the New Patient Information Packet, you will be able to fill out much  of these forms before you arrive. Our office will also try  to contact you a day or so before your visit to confirm your visit. It is very important that we confirm that you  are going to be present.

Our office will 'block off' an appropriate amount of time to discuss your concerns with Dr. Fulks.  If your plans change, please make us aware so that we can provide courteous care to our other patients.  While every effort is made to maintain our schedule, emergencies and unexpected issues are occasionally a part of a busy surgical practice.   We ask your patience if one of these instances arises.

 

  Consultation

 Patient education is an important part of making your surgical decision.  During your consultation, Dr. Fulks will explain the procedures and the aftercare in great detail to be sure all concerns of the patient are addressed.   It is critical to both the patient and the doctor that a proper understanding of the nature of the procedure, the recovery and realistic expectations be developed by the patient BEFORE SURGERY. It is the purpose of the initial consultation to thoroughly cover this information to allow patients to make educated choices concerning procedures and timing.
